Description
Anteriorly, the heart sits slightly left of midline with the ascending aorta rising superiorly to the aortic arch and continuing as the descending thoracic aorta. From the convexity of the arch, the brachiocephalic trunk courses superolaterally toward the right to divide into the right common carotid and right subclavian arteries, while the left common carotid and left subclavian arteries arise independently and ascend or run laterally to the left. Venous return is represented in blue, with the right and left brachiocephalic veins converging inferomedially to form the superior vena cava as it enters the right atrium, and subclavian and internal jugular tributaries tracking in parallel to their arterial counterparts. Attention to the initial branching pattern from the aortic arch matters because this is where anatomic variation and disease drive real consequences, from carotid territory embolic stroke to upper limb ischemia in proximal subclavian stenosis. Catheter-based access from the right radial or right brachial artery, central venous catheter placement into the brachiocephalic vein or superior vena cava, and mediastinal surgery all depend on a clear mental map of these anterior relationships.
