Description
Anteriorly, the right coronary artery arises from the right aortic sinus and courses in the right atrioventricular (coronary) sulcus toward the acute margin, while the left main coronary artery quickly divides into the left anterior descending artery in the anterior interventricular sulcus and the circumflex artery sweeping leftward in the atrioventricular groove. Diagonal branches and septal perforators radiate from the LAD across the anterior wall of the left ventricle, and right marginal branches track along the inferior border. Superficial coronary veins, including the great cardiac vein accompanying the LAD, are suggested alongside green lymphatic channels and bulbous lymph nodes clustered along the roots of the great vessels. Coronary anatomy in this projection matters because it matches the operative and angiographic mental map used to localize ischemia to a culprit vessel, for example an anterior ST elevation myocardial infarction from proximal LAD occlusion versus lateral wall ischemia involving obtuse marginal branches of the circumflex.
