Description

Seen from a posterior perspective, the stylized white myocardium is crossed by the coronary circulation highlighted in red along the atrioventricular (coronary) sulcus and the posterior interventricular sulcus. The right coronary artery courses in the right atrioventricular groove toward the crux cordis, where it commonly gives rise to the posterior interventricular artery (posterior descending artery) descending inferiorly toward the apex between the right and left ventricles. Superiorly, the great vessels cluster at the base of the heart, with the ascending aorta and pulmonary trunk positioned anterior to the left atrium, while the posterior surface is dominated by the left atrium with the pulmonary veins entering it. Posterior coronary anatomy matters because it is where coronary dominance becomes clinically legible: in a right-dominant pattern the right coronary artery supplies the posterior interventricular artery and often the atrioventricular nodal branch near the crux. That relationship explains why inferior and posterior myocardial infarction patterns can associate with AV block, and why revascularization planning for lesions at the distal RCA or proximal circumflex must account for branches that wrap onto the diaphragmatic surface. A good posterior rendering also helps clarify the spatial separation between the coronary arteries on the epicardium and the pulmonary veins entering the left atrium, a frequent landmark set during left atrial ablation procedures.
