Description

Arising from the adjacent metacarpals and filling the intermetacarpal spaces, the dorsal interosseous muscles are presented from an anterior (palmar) aspect with their bipennate fiber architecture converging toward the bases of the proximal phalanges. The interossei lie deep to the palmar aponeurosis and the flexor tendon sheath, flanking the metacarpals while the flexor tendons course distally in the midline toward the digits. Dorsally, the extensor apparatus and dorsal digital expansion relate to the interosseous insertions, but the emphasis remains on the deep intrinsic layer. Small and tight anatomy. From a teaching and surgical standpoint, this angle clarifies how the interossei contribute to finger abduction and, through their insertion into the extensor hood, to metacarpophalangeal flexion with simultaneous interphalangeal extension, the mechanical basis of the intrinsic plus posture. That relationship becomes clinically concrete in ulnar nerve palsy, where denervation of the interossei produces clawing of the ulnar digits and weakens key pinch, and in compartment syndromes of the hand where the interosseous compartments can develop dangerous pressure rises after crush injury. Hand surgeons also reference these planes during dorsal and volar approaches for metacarpal fractures and intermetacarpal space infections.
