Description
Anteriorly, the iliolumbar ligament is seen spanning from the transverse process of L5 (often with fibers from L4) inferolaterally to the iliac crest and adjacent iliac fossa, forming a short, fan-shaped band that sits deep to the abdominal wall and anterior to the posterior sacroiliac ligament complex. Medial to its iliac attachment, the anterior margins of the lumbar vertebral bodies and the lumbosacral junction provide the osseous backdrop; laterally, the ligament blends toward the inner table of the ilium near the iliac tuberosity. Superior and slightly lateral to the ligament, the iliacus occupies the iliac fossa, while the quadratus lumborum lies posterior to the transverse processes and helps frame the ligament’s proximal attachment. Stability at L5 to S1 depends on this structure more than many learners expect. By resisting anterior shear and lateral flexion at the lumbosacral junction, the iliolumbar ligament becomes clinically relevant in suspected lumbosacral transitional vertebrae, where altered mechanics can produce focal pain at the iliac crest region (often labeled Bertolotti syndrome) and complicate level identification for injections or surgery. Palpation-directed procedures and radiology reports commonly reference the iliac crest, but the ligament’s deep course explains why symptoms can mimic sacroiliac joint pain. A small structure.
