Description
Arising from the transverse process of L5, and often with fibers from L4, the iliolumbar ligament fans laterally and inferiorly to the medial iliac crest, forming distinct superior and inferior bands that bridge the lumbosacral junction to the ilium. The ligament sits posterior to the iliacus fossa and anterior to the posterior sacroiliac ligament complex, with its fibers spanning over the sacroiliac region near the iliac tuberosity. Inferiorly, the upper femoral shafts and acetabula frame the pelvic ring, while the lumbar vertebrae remain stacked superior to the sacrum. Clear landmarks. Stabilization at L4 to L5 and L5 to S1 is the point here, because the iliolumbar ligament restrains anterior translation and lateral bending of L5 on the sacrum and helps limit motion that can provoke mechanical low back pain. Clinicians encounter it when differentiating sacroiliac joint pain from lumbosacral sources, and in image guided injections where the ligament and adjacent dorsal rami can be pain generators. For surgeons, the ligament’s relationship to the L5 transverse process matters during approaches that mobilize the lumbosacral junction or expose the iliac crest, since traction or dissection in this corridor can irritate the lumbosacral trunk.
