Description

Anteriorly, the trachea descends in the midline from the larynx through the thoracic inlet, then bifurcates at the carina into the right and left main bronchi as they enter the hila on the medial surfaces of the lungs. The right lung sits slightly more superior with three lobes separated by the horizontal and oblique fissures, while the left lung shows two lobes and a clear anterior cardiac notch along its medial border adjacent to the mediastinum. Segmental bronchial branching is visible within the spongy respiratory parenchyma, framed by a transparent rib cage with the vertebral column posteriorly as a midline reference. An anterior perspective like this is the cleanest way to teach right-left asymmetry in pulmonary anatomy and to anchor airway orientation before moving to imaging. The carina and main bronchi are key landmarks in bronchoscopy and endotracheal intubation, and the more vertical, wider right main bronchus explains the common predilection for aspirated material to lodge on the right side. Cardiac impressions matter, too. The left lung’s cardiac notch and lingula shape the “silhouette sign” on frontal chest radiographs when lingular consolidation obscures the left heart border.
