Description

Centered in the anterior thorax, the larynx continues inferiorly as the trachea, its C shaped cartilaginous rings visible along the midline. At the level of the carina, the airway bifurcates into the right and left main (primary) bronchi, which diverge inferolaterally toward each lung hilum. Each main bronchus then gives rise to progressively smaller lobar and segmental bronchi, the branching pattern spreading laterally and inferiorly into the pulmonary parenchyma as bronchioles. Trachea stays medial. Bronchi angle outward. An anterior presentation like this makes the bifurcation anatomy easy to teach, and it frames a few high yield clinical details: the right main bronchus is typically wider, shorter, and more vertical than the left, which is why aspirated foreign bodies often lodge on the right and why endotracheal tubes can unintentionally enter the right bronchus if advanced too far. The carina also matters at bronchoscopy, where blunting or fixation can suggest subcarinal lymphadenopathy or mass effect, and the proximal branching guides interpretation of airway narrowing in asthma and chronic bronchitis.
