Description
Presented in anterior orientation, the male pelvis and proximal femora frame the deep gluteal compartment where quadratus femoris spans from the lateral border of the ischial tuberosity to the intertrochanteric crest of the femur. Its square muscle belly lies inferior to the obturator internus tendon and the gemelli, running laterally and slightly superiorly toward the posterior aspect of the proximal femur. Medially, the ischium and pubis define the pelvic ring, while the acetabula sit lateral to the sacrum and coccyx, receiving the femoral heads and necks shown in close apposition. Quadratus femoris matters because its insertion at the quadrate tubercle and adjacent intertrochanteric crest places it at the intersection of hip external rotation mechanics and posterior hip impingement patterns. In clinical imaging, edema or tearing at the myotendinous junction can accompany ischiofemoral impingement, where narrowing between the ischial tuberosity and the lesser trochanter compresses the quadratus femoris, often producing deep buttock or groin pain that can be mistaken for intra-articular hip pathology. This angle also helps clarify why the muscle sits deep to gluteus maximus and why surgical approaches to the posterior hip must respect the short external rotators as a layered, neurovascularly relevant plane.
