Description
Extending inferiorly from the abdominal aorta, the right and left common iliac arteries course along the medial border of the psoas region before dividing into internal and external iliac arteries at the pelvic brim. The external iliac arteries track anteroinferiorly toward the inguinal ligament, then continue distally as the femoral arteries in the femoral triangle, positioned anteromedial to the proximal femur. Branching patterns typical of the upper thigh are suggested, with profunda femoris arising posterolaterally from the femoral artery and giving perforating branches that pass posterior to the femoral shaft. Bony landmarks frame the vascular tree, including the pubis and ischium of the hip bone, the sacrum posteriorly, and the paired femora descending inferiorly. Anterior orientation matters here because the iliac to femoral transition defines a common access corridor and a frequent site of occlusive disease. The external iliac and common femoral segments are central to endovascular planning for peripheral arterial disease, and their relationship to the inguinal ligament guides where you can safely compress for hemostasis after catheterization. Groin pathology lives in this space. In open surgery, exposure of the common femoral artery and its bifurcation into superficial femoral and profunda femoris (deep femoral) arteries depends on recognizing this anterior topography and the way the profunda typically dives posterolaterally.
