Description
Centered in the anterior neck, the paired inferior thyroid arteries arise from the thyrocervical trunks and sweep medially with a characteristically tortuous course toward the posteroinferior aspect of each thyroid lobe, near the lower pole and adjacent isthmus. Their ascent tracks posterior to the carotid sheath, where the common carotid arteries and internal jugular veins lie lateral, while the trachea remains midline and the esophagus sits slightly left posterior. Strap muscles (sternohyoid and sternothyroid) overlie the gland anteriorly, and the clavicles and manubrium frame the thoracic inlet inferiorly. Tortuosity stands out. Surgical relevance is direct. During thyroidectomy or parathyroid exploration, surgeons often ligate the inferior thyroid artery branches close to the thyroid capsule to reduce risk to the recurrent laryngeal nerve, which typically ascends in the tracheoesophageal groove and crosses the artery variably before entering the larynx near the cricothyroid joint. This view also supports discussion of thyroid blood supply patterns and anastomoses with the superior thyroid artery, a common source of brisk bleeding when capsular branches are divided. An additional teaching point is anatomic variation, including asymmetry in origin from the thyrocervical trunk and occasional contributions to the parathyroid glands.
