Description
Shown from an anterior knee perspective in an adult male, the superior lateral genicular artery branches from the popliteal artery and courses laterally to sweep superior to the lateral femoral epicondyle, forming the characteristic arterial loop described in the genicular anastomosis. The vessel tracks deep to the distal quadriceps tendon and adjacent lateral retinacular tissues, then turns anteriorly toward the peripatellar plexus. Bony landmarks include the distal femur with its lateral condyle and epicondyle, the proximal tibia and fibular head laterally, and the patella positioned anterior to the trochlear groove. Veins accompany the arterial paths as paired venae comitantes. That lateral epicondylar loop is a surgical landmark. During lateral approaches to the distal femur, total knee arthroplasty exposure, or aggressive lateral release for patellar maltracking, the superior lateral genicular artery is one of the vessels most often encountered and inadvertently cauterized, and loss of this contribution can matter when combined with other disrupted genicular branches. Its anastomoses with the inferior lateral genicular artery and descending branches from the profunda femoris system also explain why periarticular collateral flow can sustain the leg when the superficial femoral or popliteal segment is compromised.
