Description
Anterior forearm anatomy is presented with the supinator highlighted deep to the superficial flexor and extensor contours, wrapping from the lateral epicondyle and supinator crest of the ulna around the proximal third of the radius. Laterally, the muscle lies deep to brachioradialis and the extensor carpi radialis longus and brevis as it approaches the radial neck, while medially it relates to the proximal ulna and the cubital fossa region. Proximally, the annular ligament and proximal radioulnar joint sit immediately adjacent, and distally the muscle fibers blend onto the lateral and anterior radius where they generate forearm supination. Bony landmarks of the radius and ulna continue to the wrist and hand, with the carpal bones, metacarpals, and phalanges positioned in anatomical orientation. This anterior perspective matters because the supinator forms a key tunnel for the deep branch of the radial nerve (posterior interosseous nerve), which passes between its heads beneath the fibrous arcade of Frohse at the proximal radius, a common site of radial tunnel syndrome and posterior interosseous nerve entrapment. Pain with resisted supination and middle finger extension testing often traces back to this relationship, and surgical decompression targets the supinator edge while protecting the nerve as it spirals posterolaterally.
