Description
Centered in the anterior neck, the paired thyroid lobes sit lateral to the upper trachea and join across the midline as the isthmus. A plexiform web of venules and capsular veins courses over the gland’s anterior surface, converging superiorly into the superior thyroid veins near the upper poles, laterally into the middle thyroid veins from the mid-lobe region, and inferiorly into the inferior thyroid veins descending from the isthmus and lower poles. The venous channels track along the capsule and superficial parenchyma, with their larger trunks running inferior and lateral toward the jugular and brachiocephalic venous pathways. Arterial supply is only partly suggested at the inferior margin. Anterior mapping of the thyroid veins matters in thyroidectomy and in tracheostomy planning, where bleeding usually comes from thin-walled venous plexuses rather than named arteries. The middle thyroid vein, when present, commonly crosses to the internal jugular vein and can tether the lobe laterally during mobilization, while inferior thyroid veins form a variable midline network that may overlie the anterior trachea below the isthmus. Small vessels. Big consequences.
