Description
Running from the pelvic brim into the proximal thigh, the external iliac vein continues as the femoral vein, which lies medial to the femur and passes deep to the inguinal region before descending toward the adductor canal. Superficially, the great saphenous vein tracks along the anteromedial leg and thigh, crossing anterior to the medial malleolus and converging superiorly toward the saphenofemoral junction, while the small saphenous vein rises from the lateral foot to the posterior calf and drains toward the popliteal vein at the knee. Distally, dorsal venous arches of the foot feed superficial tributaries over the metatarsals and phalanges, with deep paired venae comitantes accompanying the anterior tibial and posterior tibial arteries along the tibia and fibula. Bones anchor the landmarks. Femur, patella, tibia, fibula, and the tarsal and metatarsal skeleton provide clear anterior reference points. An anterior mapping like this matters when you need to teach how superficial and deep venous drainage communicate through perforating veins and where reflux tends to propagate in chronic venous insufficiency. The saphenofemoral junction and the course of the great saphenous vein are key when planning endovenous ablation, surgical ligation, or ultrasound-guided access, while the popliteal and femoral veins define the common distribution of proximal deep vein thrombosis. Bony contours around the knee and medial malleolus help correlate venous pathways with probe positioning and compression points during duplex examination.
