Description
Viewed from an inferior (abdominal) perspective, the diaphragm forms a domed musculotendinous partition with a central tendon positioned superior to the upper abdominal viscera and encircled peripherally by costal and crural fibers. The descending thoracic aorta lies posterior and slightly left of midline as it approaches the diaphragm, then continues through the aortic hiatus at the level of the crura, adjacent to the vertebral column. Inferior phrenic arteries arise near the aortic opening and course laterally and superiorly on the undersurface of the diaphragm, tracking toward the muscular periphery where they anastomose with intercostal and musculophrenic branches. Orientation from below clarifies why the aorta does not pierce the diaphragm itself but passes behind it, a key point when teaching the distinction between the caval opening in the central tendon and the more posterior aortic hiatus bounded by the right and left crura. It also frames the inferior phrenic arteries as clinically relevant collateral channels, often enlarged when supplying tumors of the hepatic dome or diaphragm and commonly encountered as feeders during transarterial embolization. Injury or unintended embolization can present as postoperative diaphragmatic ischemia or referred shoulder pain through diaphragmatic irritation.
