Description
Arcing from the pterygoid hamulus of the sphenoid to the posterior end of the mylohyoid line on the medial mandible, the pterygomandibular raphe appears as a tendinous band at the junction of the oral cavity and oropharynx. Medial to it lies the pharyngeal lumen with the superior pharyngeal constrictor forming a posterior muscular wall, while lateral and anterior soft tissues correspond to the buccinator and cheek. Inferiorly, the floor of mouth relates to the mylohyoid, and superiorly the raphe approaches the soft palate region near the hamulus. Clear landmarks. Dentists and oral surgeons use the pterygomandibular raphe as a surface guide during inferior alveolar nerve block, where needle placement targets the pterygomandibular space just lateral to the raphe, between the mandibular ramus and medial pterygoid. Misidentification can drive the needle too medially into the buccopharyngeal fascia or superior pharyngeal constrictor, increasing pain, trismus, or an intramuscular injection, and too posterior a trajectory risks proximity to the parotid region. This inferior perspective also helps teach how buccinator fibers blend with the pharyngeal constrictor across the raphe, clarifying the functional continuity between cheek and pharynx during bolus control and swallowing.
