Description
Clavicula, viewed from its inferior aspect, brings the extremitas sternalis into focus with the facies articularis sternalis forming a compact, groove-like articular facet at the medial end. Immediately lateral to this facet, the inferior surface typically transitions into the impression for the costoclavicular ligament (impressio ligamenti costoclavicularis), a roughened area that anchors the clavicle to the first rib and its costal cartilage. More laterally along the shaft, the inferior contour may hint at the sulcus musculi subclavii, running longitudinally on the underside of the bone, while the anterior and posterior borders define the torsion of the medial clavicle around its long axis. Sternoclavicular (SC) joint anatomy is easy to misjudge without an inferior reference, because the sternal articular surface and adjacent costoclavicular attachment explain why posterior SC dislocation can threaten mediastinal structures while the costoclavicular ligament often limits gross translation. Fractures near the medial third of the clavicle and physeal injuries in adolescents are also interpreted against this region, where irregular cortical texture can represent ligamentous footprint rather than comminution. Surface quality matters here, and the inferior perspective clarifies which irregularities are expected at the costoclavicular impression versus pathologic erosion seen in SC osteoarthritis or inflammatory arthropathy.
