Overview of the Spinal Cord

Description

Running inferiorly from the medulla oblongata at the foramen magnum, the spinal cord forms the central nervous column within the vertebral canal, with a subtle cervical enlargement superiorly and a taper toward the conus medullaris inferiorly. An anterior perspective typically brings the anterior median fissure into view, flanked by right and left anterolateral sulci where ventral rootlets emerge to form the anterior (ventral) roots of the spinal nerves. Segmental spinal nerves course laterally in a repeating pattern, then continue as peripheral nerves beyond the intervertebral foramina. Rostral-caudal organization is the organizing theme. Clinically, an anterior view maps directly to motor and pain-related pathways: the corticospinal tracts descend in the anterior and lateral funiculi, and the anterior spinal artery supplies the anterior two-thirds of the cord, a distribution that explains the classic weakness and loss of pain and temperature seen in anterior spinal artery syndrome with relative sparing of dorsal column modalities. Ventral root involvement at a given level produces lower motor neuron signs in the corresponding myotome, a distinction that helps separate radiculopathy from cord compression. This orientation also supports teaching Brown-Sequard patterning once laterality is introduced.
