Description
Viewed from a lateral aspect of the left heart, the epicardium is represented with emphasis on its deep connective component, the subserosa (subepicardial tela). Loose collagen bundles and elastic fibers form a compliant matrix deep to the mesothelium and superficial to the myocardium, and this stratum blends with the adventitia of coronary vessels as they course within the subepicardial space. Along the lateral wall, that connective tissue plane sits superficial to the left ventricular myocardium and extends toward the obtuse margin where epicardial fat commonly accumulates. For teaching and procedural planning, this layer matters because it is the anatomic corridor that contains the major epicardial coronaries and their venae comitantes, as well as autonomic nerves and lymphatics; surgeons and electrophysiologists encounter it during coronary artery bypass grafting, pericardiotomy, and epicardial access for ventricular tachycardia ablation. Inflammatory and fibrosing processes such as acute pericarditis, post-infarction pericardial reactions, or prior cardiac surgery can thicken or tether the subserosa, limiting normal glide between pericardium and myocardium and increasing the risk of bleeding when the subepicardial vessels are mobilized.
