Description
Seen from the left lateral aspect, the epicardium (visceral layer of serous pericardium) covers the external surface of the left ventricle and left atrium, with the subserosa (subepicardial tela) interposed deep to the mesothelium and superficial to the myocardium. Subepicardial adipose and loose connective tissue occupy this plane and commonly invest the coronary vessels and cardiac nerves as they course in the atrioventricular and interventricular grooves. The layer thickens where fat accumulates and thins toward bare myocardial contours. A subtle but real interface. Clinically, this subepicardial compartment matters because it forms the surgical and pathological corridor between pericardium and muscle, the same plane encountered during pericardiotomy, epicardial mapping, and when mobilizing epicardial fat to expose coronary arteries for bypass grafting. In pericarditis, inflammation can extend to the epicardium and into the subserosa, promoting adhesions and altering the normally gliding relationship between visceral and parietal pericardium. Epicardial adipose also carries arrhythmogenic relevance, with fat and fibrosis near the left ventricular epicardium implicated in some ventricular tachycardia substrates and in procedural planning for epicardial ablation. Use this lateral left-sided depiction when you need to teach the layered organization of the pericardium in gross anatomy and cardiothoracic modules, or when illustrating operative steps that require distinguishing epicardium, subepicardial tissue, and myocardium in a surgical atlas.
