Description
Along the left lateral surface of the heart, the epicardium (visceral layer of serous pericardium) forms the outer glistening boundary, while the subserosa (subepicardial tela) lies immediately deep to it as a loose connective tissue plane before the underlying myocardium. Adipose within this subepicardial layer thickens along the atrioventricular and interventricular grooves, where it envelopes the proximal coronary vessels and their accompanying veins. From a lateral perspective, the left ventricle dominates inferiorly and posteriorly, with the left atrial region more superior and posterior, each separated from the serosal surface by the same subserosal continuum. A sliding interface. Attention to the subserosa matters because it is the working corridor for coronary anatomy and for fluid and inflammatory spread on the cardiac surface. Pericarditis and postoperative epicardial inflammation often track along this loose tissue, and edema or fat expansion can exaggerate the apparent separation between epicardium and myocardium on gross specimens and cross sectional correlation. During coronary artery bypass grafting or epicardial lead placement, dissection stays in the subepicardial plane to mobilize the coronary arteries, avoid inadvertent myometrial entry, and preserve superficial coronary venous tributaries running within the same adipose rich layer. Use this plate in gross anatomy and cardiovascular modules to teach the layered organization of the heart surface, or in surgical and pathology publications discussing pericardial disease, epicardial fat, and coronary exposure on the left side. It also supports imaging oriented teaching when correlating the epicardial fat plane on CT and MRI with the anatomic subepicardial tela seen at dissection.
