Description
Anterior to the bladder neck, the pubic symphysis and retropubic space (cavum retzii) frame the proximal urethra as it continues inferiorly from the vesical outlet. Surrounding the lumen, the detrusor vesicae transitions into the smooth muscle of the bladder neck, with circularly oriented fibers forming the internal urethral sphincter region at the junction of bladder base and urethra. Posteriorly, the trigone and posterior bladder wall lie closer to the anterior vaginal wall, while laterally the muscular wall thickens toward the inferolateral aspects of the vesical base. Mucosa lines the lumen and lies deep to the muscularis. A compact zone. Bladder neck anatomy is where continence mechanics become clinically tangible: slight changes in detrusor tone, urethral support, or outlet configuration can shift pressure transmission and contribute to stress urinary incontinence. Surgeons mapping a retropubic approach for a midurethral sling or a Burch colposuspension rely on this relationship between the vesical outlet, proximal urethra, and retropubic tissues to avoid bladder perforation and to position fixation points at the correct level. This view also helps clarify how detrusor overactivity can coexist with outlet dysfunction, a common pattern in mixed urinary incontinence.
