Description

Anterior sectional anatomy through the adult female bladder neck brings the detrusor vesicae into focus as it thickens and funnels inferiorly toward the internal urethral orifice. Superficial to the mucosa, the smooth muscle bundles encircle the vesical neck and continue distally with the proximal urethral wall, while the trigone sits posterior and inferior to the bladder base as a smoother, less trabeculated region. Medially, the bladder lumen narrows into the urethral channel; laterally, the detrusor blends with perivesical connective tissue at the bladder wall. Clear delineation of detrusor fibers at the bladder neck matters when teaching continence and voiding mechanics, because this junction is where coordinated detrusor contraction must coincide with urethral relaxation to permit efficient urine flow. It is also where clinicians localize bladder outlet obstruction and functional obstruction patterns on urodynamics, and where detrusor overactivity can translate into urgency and urge incontinence despite an anatomically intact outlet.
