Description
Running between the lateral border of the tibia and the medial border of the fibula, the interosseous membrane of the leg spans the interosseous space as a broad fibrous sheet. From an anterior perspective, the tibial shaft sits medial and slightly anterior to the fibular shaft, while the membrane courses obliquely with its fibers generally directed inferolaterally toward the fibula. Proximally the membrane approaches the knee region beneath the level of the tibial condyles, and distally it continues toward the distal tibiofibular syndesmosis above the ankle mortise. Bony landmarks of the tibia, fibula, and adjacent ankle and foot skeleton provide orientation. Clinically, this membrane is more than a partition, it defines the boundary between the anterior and posterior compartments of the leg and forms part of the deep fascial envelope that influences compartment pressure. An anterior view supports teaching of anterior compartment syndrome, where swelling of tibialis anterior, extensor hallucis longus, and extensor digitorum longus threatens the deep fibular nerve and anterior tibial artery as they track along the anterior surface of the membrane. It also helps explain patterns of force transmission between tibia and fibula and why high ankle sprains and Maisonneuve injuries can involve disruption extending along the distal syndesmotic region and interosseous membrane. Orthopaedic anatomy courses and surgical teaching files use this plate to orient learners before approaches to tibial shaft fractures, syndesmotic fixation, and external fixation pin planning where avoidance of the interosseous space matters.
