Description

Anterior thoracic anatomy is oriented to the right lung, with the right main bronchus descending from the tracheal carina and continuing as the bronchus intermedius before giving rise to the right basal segmental bronchus. Branches course inferiorly and laterally toward the basal segments of the right lower lobe, deep to the anterior lung surface and anterior to the vertebral column. The right upper and middle lobar bronchi lie superior and anterior to the lower lobe airway, and the left bronchial tree remains contralateral and more oblique. Airways dominate the field. Clinically, the right lower lobe basal segments are frequent destinations for aspirated material because the right main bronchus is wider and more vertical, and the basal segmental bronchi guide debris into dependent lung. This anterior perspective supports teaching bronchoscopy orientation and segmental localization, including how to plan a targeted bronchoalveolar lavage or transbronchial biopsy in suspected right lower lobe pneumonia, atelectasis, or bronchiectasis. It also helps explain why right-sided postobstructive collapse can respect segmental boundaries when a segmental bronchus is occluded by mucus plugging or an endobronchial lesion.
