Centered at the proximal stomach, the cardia lies immediately inferior to the esophageal hiatus and surrounds the gastroesophageal junction where the abdominal esophagus continues into the gastric lumen. From an anterior perspective, the cardia sits superior to the fundus and medial to the lesser curvature, with the gastroesophageal junction forming a short transition zone before the body of the stomach expands inferiorly. The left margin blends toward the fundic dome, while the right margin approaches the cardial notch and the beginning of the lesser curvature. Orientation matters. Precise localization of the cardia is what you need when teaching and documenting gastroesophageal reflux disease and its sequelae, because Barrett esophagus, reflux esophagitis, and adenocarcinoma are defined relative to the gastroesophageal junction rather than to the stomach in general. Endoscopists use the top of the gastric folds and the Z line as reference points, and surgeons planning a fundoplication or hiatal hernia repair need a clear mental map of how the cardia relates to the diaphragmatic hiatus and the proximal lesser curvature where the anterior vagal trunk runs. Misidentifying the junction changes staging and margins.
