Description
Cervical intervertebral disc arthroplasty occupies the disc space between two adjacent cervical vertebral bodies, most plausibly at a midcervical level such as C5 to C6 where degenerative disease commonly concentrates. Superior and inferior metallic endplates seat against the vertebral endplates, while the central core sits between them to preserve anterior and posterior disc height. Adjacent anatomy frames the implant: uncovertebral joints lie laterally, the vertebral bodies and endplates bound it superiorly and inferiorly, and the spinal canal runs immediately posterior to the posterior longitudinal ligament. Alignment is the message. Cervical disc replacement is selected for symptomatic cervical disc degeneration presenting with radiculopathy or selected cases of myelopathy, often after failure of conservative management and when segmental instability is limited. Keeping an arthroplasty device centered in the midline and parallel to the vertebral endplates helps maintain sagittal balance and can reduce adjacent-segment overload compared with anterior cervical discectomy and fusion, while malposition can contribute to facet joint pain, persistent foraminal stenosis, or heterotopic ossification.
