Description
Anterior projection of the left lung highlights the apicoposterior segment of the superior lobe (segmentum apicoposterius, S1+S2) in distinct color against the remaining upper-lobe parenchyma and the inferior lobe. Superiorly, the segment reaches the apex under the cervical pleura, while inferiorly it tapers toward the oblique fissure and the hilum. Medially, its bronchovascular supply tracks toward the left lung root, where the left main bronchus and pulmonary artery pass posterior and superior to the pulmonary veins in the classic hilar arrangement. Clear boundaries. Segmental anatomy matters because pathology and intervention often respect these planes. The apicoposterior segment commonly hosts post-primary tuberculosis and cavitary disease due to higher regional ventilation, and it is a frequent target in thoracoscopic segmentectomy when a small peripheral nodule lies in the upper lobe but lobectomy would be excessive. Knowing that S1 and S2 are often fused on the left helps reconcile differences between bronchoscopic segment mapping and CT-based planning, and it explains why the apicoposterior bronchus and artery may be approached as a combined pedicle.
