Description

Centered on the proximal large intestine, the male cecum (caecum) appears as a blind-ended pouch in the right iliac fossa, continuous superiorly with the ascending colon at the ileocecal junction. The terminal ileum approaches the cecum from a superomedial direction, entering at the ileal orifice where the ileocecal valve forms a mucosal fold that projects into the cecal lumen. Along the inferior aspect, the typical sites of convergence of the taeniae coli indicate the expected origin of the vermiform appendix, while the cecal base lies inferior to the ileocecal junction and medial to the lateral paracolic gutter. Orientation matters. Cecal morphology is more than a shape study because its position and peritoneal attachments determine how pain localizes and how disease spreads. Acute appendicitis often begins with vague periumbilical pain and then migrates to right lower quadrant tenderness when the parietal peritoneum near the cecum becomes involved, and a retrocecal appendix can blunt classic signs and complicate ultrasound windows. Surgeons also use the cecum as a landmark during colonoscopy to confirm complete intubation, identifying the ileocecal valve and appendiceal orifice to avoid mistaking a redundant colon for the cecal caput.
