Description

Framing the initial segment of the adult male large intestine, the cecum appears as a blind-ended sac in the right iliac fossa, continuous superiorly and medially with the ascending colon at the ileocecal junction. The terminal ileum approaches from the left and posterior aspect to enter the cecum through the ileal orifice, where the ileocecal valve (valva ileocaecalis) forms mucosal lips that project into the lumen. Inferiorly, the vermiform appendix arises from the posteromedial cecal wall below the ileocecal junction, and the three taeniae coli converge toward its base. Peritoneal relationships and the nearby ileocecal fold complete the regional orientation. Cecal anatomy matters because small differences in position and fixation change both symptoms and surgical strategy. Appendicitis frequently presents with variable tenderness when the cecum is mobile or the appendix is retrocecal, and awareness of the taeniae coli as a tactile guide to the appendiceal base remains practical in open and laparoscopic appendectomy. The ileocecal valve is another teaching landmark, helping distinguish small bowel from large bowel during colonoscopy and informing the pattern of Crohn disease involvement around the terminal ileum and cecum.
