Description
Arising from the superior border of the thyroid isthmus, the pyramidal lobe (lobus pyramidalis, Lalouette’s lobe) extends cranially in the midline or slightly paramedian, most often toward the left of the trachea. Its fibrous continuation, the levator glandulae thyroideae, may track superiorly toward the hyoid bone, lying deep to the infrahyoid strap muscles. Flanking this accessory process, the right and left thyroid lobes sit anterolateral to the upper tracheal rings with the isthmus bridging them anteriorly. Persistence of the thyroglossal duct explains the pyramidal lobe, and that embryologic detail becomes practical in the operating room. During total thyroidectomy for Graves disease or differentiated thyroid carcinoma, residual pyramidal tissue can be a source of persistent hyperthyroidism or a focus for recurrent disease if it is not identified and excised, and its superior extent may be missed when exposure stays confined to the level of the isthmus. Surgeons often follow the tract toward the hyoid, working between or through the sternohyoid and sternothyroid, while staying oriented to the midline and the prelaryngeal (Delphian) lymph node region. Endocrine anatomy teaching benefits from a dedicated rendering of this variant because it reinforces that the thyroid is not always a simple bilobed organ with an isthmus.
