Description
Posteriorly oriented anatomy of a single thyroid gland lateral lobe is presented with its medial surface facing the visceral compartment of the neck, where the trachea lies anteromedial and the cervical esophagus sits posteromedial. A thickened posteromedial contour suggests the region where the gland approaches the tracheoesophageal groove. Superiorly, the lobe tapers toward the area of the superior pole, while the inferior pole descends toward the thoracic inlet. This posterior perspective matters because it maps the operative and teaching corridor that contains the recurrent laryngeal nerve and the inferior thyroid artery as they course near, and often within, the tracheoesophageal groove before entering the larynx. Small variations in how the thyroid capsule and Berry ligament tether the gland to the laryngotracheal framework change how the lobe rotates during medial traction, which is when recurrent laryngeal nerve injury most often occurs in thyroidectomy. The posterior surface is also where parathyroid glands are typically sought, with the superior parathyroid commonly found near the cricothyroid junction and the inferior parathyroid more variable and inferior.
