Description
Posteriorly, the paired thyroid lobes flank the upper trachea and inferior larynx, their medial surfaces approaching the cricoid cartilage and first tracheal rings. Rising superiorly from the isthmus, an accessory pyramidal lobe (lobus pyramidalis, Lalouette pyramid) extends in the midline or slightly paramedian toward the hyoid region, narrowing as it ascends. From this orientation, its relationship to the posterior gland surface is clear, with the pyramidal process projecting superiorly while the main lobes broaden inferolaterally along the tracheoesophageal grooves. A midline remnant of the thyroglossal tract is implied. Recognition of the pyramidal lobe matters in thyroidectomy. Residual pyramidal tissue is a common source of persistent thyroglobulin and recurrent functioning thyroid tissue after total thyroidectomy, and it can also take up radioiodine, complicating postoperative surveillance and ablation planning. This posterior outline helps teach why surgeons often trace the superior border of the isthmus toward the hyoid and why preoperative ultrasound and scintigraphy should include the midline suprahyoid track when thyroid tissue seems discordant with expected gland volume. Small structure, large consequence.
