Seen from the anterior aspect, the female ala nasi forms the lateral wall of each naris, curving inferiorly to meet the nasal sill and medially approaching the columella at the level of the columellar-lobular junction. The alar rim outlines the external narial margin, while the alar crease marks the transition between the mobile ala and the more fixed cheek skin lateral to the nasolabial region. Beneath the skin envelope, the contour implies the underlying major alar cartilage (greater alar cartilage) with its medial and lateral crura shaping the lobule and alar dome, and the fibrofatty tissue that rounds the alar lobule inferiorly. Anterior alar anatomy matters in rhinoplasty planning because small changes in alar rim position and alar base width alter both airway function and facial balance, and the external landmarks guide where surgeons can safely place incisions for alar base reduction. It is also the key external reference for diagnosing and documenting external nasal valve compromise (alar collapse), often worsened after excessive cephalic trim of the lateral crus or in patients with weaker cartilaginous support. Subtle asymmetry is common. Clinically relevant.
