Description
Anterior cutaway anatomy of the adult female urinary bladder exposes the vesical wall in cross-section, with the mucosa and submucosa deep to the lumen and the detrusor muscle arranged in interlacing inner longitudinal, middle circular, and outer longitudinal layers. Superiorly, the bladder dome (apex) rises toward the peritoneal reflection, while the base (fundus) sits posteriorly where the trigone forms a smooth region between the ureteric orifices and the internal urethral orifice. Inferiorly, the bladder neck narrows into the proximal urethra, and the thickening of circular smooth muscle at this junction relates to the internal urethral sphincter mechanism. Wall stratification reads clearly. This view matters when you are teaching why the bladder behaves differently from most hollow viscera: detrusor fibers do not run as tidy, separable bands, so coordination during storage and voiding depends on functional layering rather than discrete muscles you can “peel apart.” Clinically, detrusor overactivity and impaired compliance in neurogenic bladder (for example after spinal cord injury or multiple sclerosis) map directly to changes in these smooth muscle layers and their autonomic innervation, and they influence urodynamic findings such as reduced capacity, high storage pressures, and urge incontinence. The trigone is a key landmark.
