Description
Posteriorly oriented, the visceral surface of the spleen is presented with the renal impression as a flattened, slightly concave area along the inferomedial portion of the organ. Medial to this contact zone, the hilum region corresponds to the entry and exit of the splenic artery and vein, while the diaphragmatic surface curves laterally and superiorly toward the convex outer contour. The renal impression lies inferior to where the gastric impression would be expected and posterior to the colic relationship along the lower pole. A clean topographic landmark. Clear identification of the renal impression matters when teaching left upper quadrant relationships and when interpreting cross sectional imaging, where the spleen and upper pole of the left kidney sit in close apposition. In blunt abdominal trauma, perisplenic hematoma or capsular disruption can track along perirenal spaces, and appreciating the posterior splenorenal interface helps radiologists and trauma surgeons distinguish splenic injury from renal or adrenal sources on CT. During splenectomy or mobilization of the spleen, this is also where dissection approaches the splenorenal ligament and tail of the pancreas near the hilum, a frequent site of iatrogenic injury. Ideal for gross anatomy practicals, abdominal anatomy lectures, and surgical atlases that need a precise depiction of the spleen’s visceral topography, including the renal impression (impressio renalis lienis) and adjacent hilar region.
