Description
Curled in the characteristic fetal flexion of the second trimester, the conceptus is rendered with the head in anterior flexion, the thoracic spine and hips flexed, and the knees drawn toward the abdomen. Facial contours are readable in profile, with the cranial vault proportionally large relative to the trunk, and the upper limbs held close to the chest. A helically twisted umbilical cord extends from the fetal umbilicus to a discoid placenta positioned superiorly in the field, its chorionic plate and lobulated maternal surface suggested by the textured, dark red mass against a black background. Around gestational week 21, this posture and proportion set the visual baseline for counseling and for correlating biometric measurements with normal development, including biparietal diameter, head circumference, abdominal circumference, and femur length. The prominent cord and placental attachment also support teaching around fetoplacental circulation, and where pathology starts to matter clinically: abnormal cord insertion (marginal or velamentous), true knots, or reduced coiling can be discussed alongside growth restriction and intrapartum risk. Small, smooth external features remain expected at this stage. Fat is still limited.
