Description
Ovarian gross anatomy is presented with the outer cortex encircling a central medulla, reflecting the classic cut-surface organization of the adult female gonad (ovarium). The tunica albuginea lies immediately deep to the surface epithelium, while the cortical stroma contains follicles at different stages and, depending on the cycle, a corpus luteum or corpus albicans. Centrally, the medulla carries the ovarian vessels within loose connective tissue that continues to the hilum, where neurovascular structures enter and where the mesovarium attaches on the medial aspect. That cortex-to-medulla contrast matters in daily practice. Follicular development, luteinization, and cyst formation are cortical processes, and this view helps anchor why common lesions such as functional follicular cysts, corpus luteum cysts, and endometriomas are described by their relationship to the ovarian stroma rather than the hilum. Surgical planes in oophorectomy or cystectomy also depend on recognizing the medullary vascular core so you can anticipate bleeding risk near the hilum and preserve ovarian tissue when fertility-sparing technique is planned.
