Description
Oriented in anatomical position, the ovary (ovarium) appears as an ovoid gonad with a subtly irregular cortical surface, and the ovarian hilum sits along its mesovarian border where the peritoneal covering reflects to form the mesovarium. Medially, the hilum faces toward the uterine side, while the free lateral pole points toward the pelvic wall. The superior (tubal) pole lies closer to the uterine tube and its fimbriae, and the inferior (uterine) pole aligns toward the utero-ovarian ligament. A focused landmark. Attention to the hilum matters because this is where the ovarian artery and vein, lymphatics, and autonomic nerves enter and leave the organ, then branch within the medulla beneath the cortex. Surgical planes during oophorectomy, ovarian cystectomy, and salpingo-oophorectomy often respect the mesovarian attachment, and traction or coagulation near the hilum can threaten the infundibulopelvic (suspensory) ligament pedicle and ovarian venous outflow. Lymphatic drainage from this region tracks along the ovarian vessels to para-aortic (lumbar) nodes, a staging point in epithelial ovarian carcinoma.
