Description
Seen from the side, the maternal torso is rendered semi-transparent so that the gravid uterus fills the lower abdomen and extends well above the umbilicus. Within the amniotic cavity a fetus of about 27 weeks lies in cephalic presentation, the head engaged toward the pelvic inlet, spine curved along the anterior uterine wall, and the limbs flexed against the trunk. The coiled umbilical cord passes from the fetal abdomen across the amniotic space, while the myometrium, cervix and vagina are visible inferiorly. The maternal urinary bladder sits anterior to the lower uterine segment, with the sacrum and lumbar vertebrae ghosted posteriorly to establish the pelvic frame of reference. By the third trimester this lateral projection makes clear the mechanical relationships that dominate late pregnancy: uterine displacement of the bowel and stomach, compression of the bladder, and the lumbar lordosis that accompanies a forward shift in the centre of gravity. The orientation is the one used when teaching fetal lie, presentation and station, and it corresponds to what is assessed by Leopold manoeuvres and by ultrasound biometry at this gestation. Week 27 also marks the transition into the third trimester, a point of interest for discussions of viability, fetal growth surveillance and the anatomy relevant to preterm labour and cervical assessment.
