Description
Seen from the side, the maternal torso is rendered semi-transparent so the gravid uterus fills the abdomen from the pelvic inlet to just below the costal margin. Within the amniotic cavity the fetus lies in cephalic presentation, head low against the pelvic brim, spine curved along the anterior uterine wall, with knees flexed and arms drawn toward the chest. The umbilical cord loops from the umbilicus toward the placental attachment on the uterine wall, while the compressed large intestine, the urinary bladder anteroinferior to the uterus, the pelvic bone and the lumbar vertebral column indicate how maternal viscera are displaced at this stage. The maternal breast and abdominal wall complete the surface contour of the late third trimester. Week 35 sits in the period when fetal lie and presentation become clinically decisive. A lateral projection of this kind shows the relationship between the fetal head, the pelvic inlet and the maternal sacrum that underlies Leopold manoeuvres, engagement assessment and decisions about mode of delivery.
