Description
The maternal torso is shown in profile with the abdominal wall rendered translucent to expose the gravid uterus filling the abdominopelvic cavity at approximately 33 weeks of gestation. The fetus lies in cephalic presentation, the head directed toward the pelvic inlet and the flexed spine following the curve of the anterior uterine wall, with limbs drawn in against the trunk within the amniotic sac. The umbilical cord loops from the fetal abdomen toward the placental surface, while the thickened myometrium, the closed cervix and the vaginal canal are visible inferiorly. Maternal intestines are displaced superiorly and posteriorly, and the urinary bladder and rectum are compressed against the bony pelvis. By the third trimester the relationship between fetal presentation and the maternal pelvis governs much of obstetric management. A lateral projection makes the longitudinal lie, the degree of engagement and the descent of the presenting part immediately legible, and it explains the familiar late-pregnancy symptoms of urinary frequency, reflux and lumbar lordosis with pelvic girdle strain. The same anatomy underpins Leopold manoeuvres, fundal height measurement, assessment of malpresentation, and counselling on external cephalic version when the fetus is not vertex. Cervical length and the closed internal os are also relevant to discussions of preterm labour risk at this stage.
