Seen from the side with the abdominal wall rendered translucent, the gravid uterus fills the abdominopelvic cavity at approximately 33 weeks of gestation, its fundus reaching well above the umbilical level. The fetus lies in cephalic presentation, head directed toward the pelvic inlet, spine curved along the maternal anterior abdominal wall, with flexed hips and knees drawn toward the chest inside the amniotic sac. The umbilical cord loops anteriorly from the fetal abdomen toward the placental implantation site on the uterine wall. Maternal structures displaced by the enlarged uterus are visible in the same cut: coils of small and large intestine pushed superiorly and posteriorly, the compressed urinary bladder anteroinferiorly, the rectum posteriorly, and the lumbar vertebrae, sacrum and pelvic bone framing the posterior contour. One hand rests on the fundus, the other supports the lumbosacral region. This lateral projection is the standard reference for teaching fetal lie, presentation and station in the third trimester, when clinicians assess whether the presenting part is vertex and whether the head has begun to engage in the pelvic brim. It also demonstrates the mechanical basis of common late-pregnancy complaints: bladder compression with urinary frequency, upward displacement of the diaphragm and bowel, gastroesophageal reflux, and increased lumbar lordosis with low back and pelvic girdle pain. The relationship of the fetal head to the sacral curve and pubic symphysis underpins pelvimetry concepts and discussions of vaginal versus operative delivery.
