Description
A lateral view of the maternal torso is rendered semi-transparent so that the gravid uterus occupies the lower abdomen and displaces the abdominal wall anteriorly. Inside the amniotic sac the fetus at approximately 25 weeks lies in a flexed, head-down attitude, the fetal head directed toward the maternal pelvic outlet and the limbs drawn close to the trunk. The umbilical cord loops from the fetal abdomen toward the placenta seated against the uterine wall, while the maternal urinary bladder and the outlines of the pelvic bones are visible inferiorly. One maternal hand rests on the upper abdomen and the other on the hip, giving scale to the fundal height at this stage of gestation. By the mid second and early third trimester the fetus has reached the threshold of viability, and clinicians use exactly these relationships when interpreting fundal height, fetal lie and presentation on Leopold manoeuvres or ultrasound. The lateral projection shows why upward pressure of the fundus on the diaphragm and forward compression of the bladder produce maternal dyspnoea, urinary frequency and lumbar lordosis with low back pain. Placental position relative to the uterine wall and cervix, and the course of the umbilical cord, are the features assessed during routine growth scans and when planning management of preterm labour.
