Description

The fetus lies curled in the amniotic cavity with the head directed inferiorly and to the left of the frame, the vertebral column convex posteriorly and the limbs drawn toward the trunk in the characteristic flexed fetal attitude. Surface features typical of the late second trimester are visible: a large cranial vault relative to the trunk, a well formed auricle with developing helix and antihelix, thin translucent skin over the shoulder and thigh, and a foot with clearly separated toes resting against the upper abdomen. The umbilical cord passes from the umbilicus toward the placenta, its coiled umbilical vein and paired umbilical arteries indicated by the spiralling vessels within the cord. Behind the fetal back the placental disc is seen in section against the myometrium, showing villous tissue and chorionic vessels along the fetal surface. At around 23 weeks the fetus sits at the threshold of extrauterine viability, making this period central to counselling on periviable birth, antenatal corticosteroid timing and neonatal intensive care planning.
