Description
Seen from an inferior perspective, the right lower lobe bronchial tree is oriented with the posterior basal segmental bronchus (B10) coursing posterolaterally toward the diaphragmatic surface. The bronchus arises from the inferior lobar (lower lobe) bronchus distal to the takeoff of the superior segmental bronchus (B6) and the adjacent basal segmental branches, then tracks into the most caudal, posterior parenchyma. Proximal cartilaginous rings give way to smaller conducting airways as the branch tapers distally. Deep territory. Gravity makes this anatomy clinically relevant. Secretions and aspirated material often settle in dependent lung, and in a supine patient the posterior basal segments of the lower lobes are frequent targets, so understanding B10 helps when correlating a posterior basal consolidation on CT with the segment that should be sampled or drained. Bronchoscopic navigation also depends on appreciating how the basal segmental bronchi diverge from the lower lobe bronchus, since misidentifying B10 can lead to incomplete bronchoalveolar lavage, missed endobronchial lesions, or poorly directed endobronchial valve placement for targeted atelectasis. Pulmonology and thoracic surgery teaching files use this view to explain segmental anatomy of the right lung, including how the inferior orientation of basal bronchi maps to dependent disease patterns and patient positioning. It also fits well in radiology atlases and bronchoscopy manuals that pair airway branching with axial and coronal CT correlations for right lower lobe pathology.
