Viewed from a superior perspective into the right lower lobe, the posterior basal segmental bronchus (B10) is centered within the posterior and inferior lung parenchyma, branching distally from the right lower lobe bronchus after the superior segmental bronchus (B6) and the basal trunk take off. Its lumen courses posteriorly and laterally toward the diaphragmatic surface, with immediate segmental subdivisions fanning into the posterior basal bronchopulmonary segment. Adjacent basal segments (lateral, anterior, and medial) sit more anterolateral or anteromedial in the same lobe, while the oblique fissure lies superior and lateral, separating lower from upper lobe structures. Orientation of B10 matters when you are localizing pathology to a bronchopulmonary segment rather than a whole lobe, because posterior basal disease often pools dependently and can be missed if you think only in lobar terms. This bronchus is a practical target in bronchoscopy for suctioning tenacious secretions, retrieving foreign material, or sampling lesions that track along the posterior basal segment, and it also guides segmentectomy planning when sparing uninvolved basal segments.
