Description
Arising from the inferior lobar bronchus of the right lung, the basal segmental bronchus is rendered from a superior perspective as it enters the basal pyramid of the lower lobe. From this vantage, the lumen and cartilaginous wall are seen dividing early into posterior and lateral trunks, oriented posterolaterally relative to the bronchus intermedius and inferior to the origin of the superior segmental bronchus (B6). Segmental branching follows the expected basilar pattern, with divisions directed toward the posterobasal (B10) and lateral basal (B9) territories. Spatial relationships read cleanly: proximal airway centrally, distal segmental bronchi radiating peripherally. Understanding this branching matters when you need to localize disease to a bronchopulmonary segment rather than to an entire lobe. Aspirated material and post-obstructive infection often track into dependent basilar segments of the right lower lobe, and the orientation of posterior and lateral trunks helps explain why posterior basal involvement is frequent in supine patients. Bronchoscopically, knowing where B9 and B10 typically arise reduces sampling error during BAL, brushings, or transbronchial biopsy, and it supports accurate correlation with CT where the right lower lobe basal bronchi can appear crowded in the hilum-adjacent slices.
