Viewed from an inferior (basal) perspective, the left lower lobar bronchus (bronchus lobaris inferior sinister) divides into tertiary (segmental) bronchi, with the basal segmental bronchus (bronchus segmentalis basalis) directed toward the diaphragmatic surface of the inferior lobe. Segmental branches course distally and laterally to supply the basal segments, while a more superiorly placed branch typically corresponds to the superior segment (S6) bronchus. Cartilaginous plates support the proximal segmental airway, and the lumina narrow as they progress toward the subsegmental bronchi. For bronchoscopy and thoracic surgery, inferior orientation matters because the basal segmental bronchi sit at the dependent portion of the lung, a common destination for aspirated material and pooled secretions in supine patients. This projection also helps clarify the segmental map that underpins segmentectomy planning, where the surgeon follows a specific segmentalis bronchus to define resection planes while preserving adjacent segments of the left lower lobe.
